Health IT QI Network
To improve prevention, HQSI will gather physician practices to focus on effective use of electronic health records for clinical decision support and clinical quality improvement. The Health IT QI Network will share best practices in patient and family engagement to identify patients who are lost to follow-up, have gaps in care, or have poorly controlled risk factors.

- Assistance with using EHRs for care coordination, monitoring, patient engagement, spread of best practices, and identification in disparities of care
