Improving Individual Patient Care

Improving the Quality of Life in Long Term Care Facilities

HQSI’s work in New Jersey focuses on improving residents’ quality of life and reducing healthcare costs in long-term care. To accomplish this, HQSI is working with long-term care facilities to:

  • Achieve a 20% relative improvement rate in pressure ulcers by June 2013
  • Reduce the use of physical restraints to 3% by June 2013

We encourage long-term care facilities to participate in the Nursing Home Quality Care Collaborative, a free quality improvement initiative that offers peer-driven learning and action activities in areas of safety, outcomes, prevention, culture, staffing, and finance.   • Access to best practices and strategies of high performing nursing homes
  • Quality Assurance Performance Improvement (QAPI) tools and resources
  • A change package focused on the successful practices of high performing nursing homes

Making Care Safer

By improving safety for every patient, healthcare providers are contributing to a 40% national reduction in healthcare-acquired condition rates. HQSI is helping nursing homes achieve this goal by partnering with the New Jersey Local Area Network for Excellence and by facilitating learning and action activities—to help ensure that every nursing home resident receives the highest quality of care. HQSI has the experience and expertise to assist long-term care facilities in their efforts to greatly improve lives.
