Healthcare Quality Strategies, Inc., (HQSI) is providing information and resources on reducing the use of potentially inappropriate medication and drug-on-drug interactions that may be beneficial to home health agencies.

HQSI is working across multiple provider settings, including home health agencies, in a southwestern New Jersey community to improve patient transitions between care settings and reduce unnecessary hospitalizations. HQSI is working with home health agencies and other provider settings to implement interventions that result in improved coordination of care and communication through system-wide changes. Interventions address issues (medication reconciliation and management, discharge planning, post-discharge follow-up, etc.) and target conditions associated with preventable re-hospitalization, such as heart failure and pneumonia.
