Filing a Complaint

Quality of Care Concerns

If you or your caregiver has a concern about the quality of medical care you have received or are receiving, call HQSI at 1-800-624-4557. If you or your caregiver would prefer to submit a written complaint, complete the
Medicare Quality of Care Complaint Form, and mail the form to Healthcare Quality Strategies, Inc., 557 Cranbury Road, Suite 21, East Brunswick, NJ 08816, or fax the form to 732-432-5638.

Examples of quality of care concerns include:

  • Wrong medication given or overdose of medication
     
  • Unnecessary surgery or diagnostic procedure performed
     
  • Surgery performed on the wrong part of your body
     
  • Delay in service affecting your health outcome
     
  • Inadequate care or treatment by a Medicare hospital or doctor
     
  • Early or premature discharge
     
  • Change in condition that was not treated or diagnosed
     
  • Inadequate discharge instructions or follow-up
     
  • Situations where you believe harm occurred
